Quality through Collaboration: Planting the Seeds for Rural Health Care
This paper is designed to introduce rural practitioners to a model for continuously improving quality in the primary care setting. The Health Resources and Services Administration (HRSA) Health Disparities Collaboratives (HDC) is supported by HRSA’s Bureau of Primary Health Care (BPHC), and they were developed with the Institute for Healthcare Improvement. Primarily, this model has been utilized in the Consolidated Health Center (CHC) Programs. It has had such successful results that several communities have adapted the model to non-CHC settings. Since the founding of our country, rural Americans have always responded when our nation has gone to war. In the American Revolution, rural Americans left their homes and their families to protect their families and their lands. During the American Civil War, rural Americans again responded to preserve their way of life, and to protect their families.
